Pelvic Health Pre-Screening


Please answer the questions below as best you can. This will give us a clearer picture of what’s going on and help us see whether a pelvic floor assessment could be useful for you.

1. Do you experience pain, tightness, or discomfort in any of the following areas?

  • Perineum

  • Testicles

  • Penis

  • Lower Abdomen

  • Inner Thighs

  • Rectal Area

2. Do you notice any urinary symptoms such as:

  • Urgency (sudden urge to urinate)

  • Frequency (urinating more than usual)

  • Hesitancy (difficulty starting the stream)

  • Post-void dribbling (leaking after urination)

3. Do you experience any of the following sexual symptoms?

  • Premature ejaculation

  • Erectile difficulties (e.g. sustaining or achieving an erection)

  • Reduced genital sensation during sex

4. Do you feel pelvic pressure, heaviness, or discomfort when sitting for long periods, during exercise, or after physical activity?

5. Have you ever been diagnosed with any of the following?

  • Prostatitis (acute or chronic)

  • Chronic Pelvic Pain Syndrome

  • Pelvic Floor Dysfunction

6. Do you frequently feel stressed, anxious, or like you’re operating in a constant "fight-or-flight" mode?

7. Do you tend to breathe primarily from your chest (shallow breathing), rather than from your abdomen (diaphragmatic breathing)?

8. Are you open to undergoing a comprehensive pelvic floor assessment, which may include external and/or internal examination, breathing pattern analysis, and muscle function testing in your pelvic area?
